Patient Communication

Opt-in and opt-out

At Phanova, we are committed to maintaining your privacy and avoiding spamming. This page explains how we collect your consent (opt-in) to receive communication from us and how you can revoke (opt-out) this consent.

Phanova Inc. provides your healthcare provider with services and tools to better manage your chronic care management (CCM) plan. We use text messaging and phone calls to help your doctor communicate with you. You are in control of this communication - we ask for your agreement before sending you messages, and you can stop getting messages at any time. If you have questions, contact support@phanova.com. You can also refer to our Terms of Service and Privacy Policy to learn more about your rights and privacy.

Opt-In Process (Joining)

In your visit, staff will ask if you want to receive text messages about your Chronic Care Management (CCM) using these words:

"As part of our Chronic Care Management service, we may send you important reminders and updates by text message or phone calls via our vendor, Phanova Inc. These texts and calls will be sent daily to monthly, so 1-30 times per month (per your physician’s discretion), and may include care reminders, questions, check-in messages, or information about your care plan. Standard text messaging and data rates may apply. You can opt out at any time by texting STOP to [number]. Do you consent to receiving these text messages and phone calls?"

Consent Documentation

Your verbal consent will be documented in the electronic health record (EHR) or patient chart at the time of enrollment. Staff will add a note:

"The patient provided express verbal consent for SMS notifications on [date]."

Confirmation Message

After giving verbal consent, you will receive a confirmation message:

Phanova: You are now opted in to receive messages about your chronic care management program in the frequency determined by your physician (1 to 30 times a month). Reply HELP for help, STOP to unsubscribe. Message & data rates may apply. Privacy Policy and Terms of Service: https://phanova.com/company-policies

Getting Help

You can always text HELP to our phone number to receive help about Phanova Inc.'s messaging services.

Opt-Out Process (Leaving, stopping messages)

You may stop messages at any time by texting STOP.

Confirmation of Unsubscription

You will receive a final confirmation message:

Phanova: You have successfully unsubscribed and will no longer receive CCM notifications. Reply START to rejoin.

Your healthcare provider will coordinate with you an alternative communication channel and your continuing participation in the programs.
